Ordering a Leaf is Easy...

• You may choose up to 5 lines of text on the leaf that you purchase.

• You may choose up to 25 letters & spaces per line. (This is contingent upon the size of the letters, i.e., “W” & “M” take up more space than other letters).

• When completing the form, please print clearly.

• You can make payment in full of $300, or you may make three $100 payments.

• Your leaf will be ordered once paid in full.
